2) Narrative Statement and Budget
Please provide the following information within a narrative statement not more than two single-spaced pages:
Please provide a separate one-page project/program budget (income and expenses) along with the narrative.
3) Financial information
Please provide the following information within a narrative statement not more than two single-spaced pages:
- A brief history of the organization, applicant artist, or company's career including major accomplishments
- A description of the proposed project or program and how it relates to your ongoing mission and artistic goals
- Brief bios of the artists and other key project/program personnel
- The timeline in which the project or program will take place
- Where it will take place
- Any challenges you anticipate and how you plan to address them
- Should the project or program extend beyond the Foundation's funding period, your strategies for raising additional funds
Please provide a separate one-page project/program budget (income and expenses) along with the narrative.
3) Financial information
- A copy of IRS letter determining 501(c)(3) tax status
- Applicant's or fiscal sponsor's most recent IRS 990 tax return
- Applicant's or fiscal sponsor's most recent audited financial statement
